Armies can be simulated by having AI NPC soldiers in battle commanded by players. How it works is similar to Mount and Blade reinforcement pool that continuously spawns soldiers as they get killed.
This has the advantage that even there is an imbalance between players in battle with AI Soldiers under the effective control of players. So for example in a battle between 10 players on one side and 20 player on the other. The 10 player team can be given 50 AI soldiers while the 20 player team is geven 30 to even things out. How things are balanced is AI armies have a force value with players having a higher value then AIs.

A force of 1000 vs 500 means the 1000 force in battle is given the advantage even if there is 10 players vs 30. In this case they are give a large number of AI soldiers while the 30 players might not be given any until the forces balance themselves out.
Cities are a what generates this army forces and are able to be deployed all over them map. They can be developed by investing resources like in a RTS game and unlock better AI soldiers and more advanced classes for players. Cities might also have an upkeep in resources based on the level of development like in the Anno 1404 series. They could generate unrest and spawn bandit forces that some players might take advantage of.
To capture a city you require a large army force that will build siege equipment. Players do not have much power against fortifications, they cannot blow up walls, they cannot destroy gates and towers can nuke players to smithereens so it requires armies.
Armies move slowly over the map and defending players are aware whenever an army is approaching and can engage them in field battles. This shifts the initiative you find In PVP games away from the attacker to the defender, with army forces being vulnerable.
There is also a supply and logistic dimension. An army has an upkeep in supplies so raiding supply routes can be an effective strategy.
The army can itself take in an amount of supplies but that will slow down its mobility and in battle would have to defend their supplies making them more defensive.
This need for supplies can have interesting effects on the economy,trade and city development. More developed cities can generate and support bigger armies but they themselves are more vulnerable to their own resource upkeep.
Players can permanently die through two methods. Either against Assassins were the assassin is at the disadvantage in enemy territory. Or through Duels which are 1vs1 PvP matches where 2 players can agree to the challenge. In a duel they cannot be affected by anything else and are healed to full.
Players can gain merit which is specific to the kingdom and fame which is global to the game.
With increase merit your rank can be higher and be eligible to more positions in your faction. Merit also accounts to the amount of AI NPCs you control in battles and the amount of force you represent.
You can also invest your merit behind another person to boost their political power.
Merit is a currency of the player, it is not a currency the current leader controls. With merit players have real power to change things in your faction.
Players can have classes with powerful abilities that can change the game significantly like RTK 13 prestige classes. How they work is by balancing control between what faction leaders control and high fame and merit can control. If you have an ability to 'open the city gates' the game will open the gates regardless of what the leader wants. Of course while you are a danger you can also provide benefits and that merit and fame was not given for free, to some extent you have been 'used' by your leader.
To some extent it's all about relationships, which can give rise to some interesting actual Role Play as your power is real.
Now the most interesting is how you can use all this things for betrayal and intrigue.
With enough merit accumulated between a group of people they can trigger a coup and army will be split based on merit. This can happen anywhere, even in the middle of a battle so there is opportunity in internal conflict.
Assassination and Duels means the merit of the killed is gone and killing a Ruler means there is a scramble on who is going to be the next ruler with breakdown into civil war fairly likely.
The bigger the empire the more volatile the elements in it. It makes for an interesting dynamic game.
